Changes Estrogen Can Have on Intimate Parts
and Sexuality

Estrogen not only affects physical appearance, but can also change the way sexuality, intimacy, and erotic sensations are experienced. Hormone therapy may directly affect intimate parts, libido, arousal, orgasm, and fertility. These changes happen in most cases, however every body responds differently and experiences with Estrogen can vary from person to person.

Lower Libido

(usually begins 1–3 months after starting HRT)

  • Interest in sex may decrease over time

  • Spontaneous arousal may become less frequent

Erections

(usually noticeable within 1–3 months)

  • Erections usually become less frequent

  • They may not feel as firm or last as long as before

  • Some people may lose the ability to penetrate

  • Because erections happen less often, they can sometimes feel uncomfortable or painful

  • Medications such as Viagra may help maintain erectile function for those who want it

Erotic Pleasure and Arousal

(more noticeable around 3–6 months)

  • Sensitivity across the body may shift over time

  • Areas that once felt highly sensitive may become less stimulating, while new sensitive areas may develop

  • Arousal may feel different and intimacy may need to be approached in new ways

Orgasm

(often stabilises around 4–6 months)

  • Orgasms may feel more like a “full body” experience rather than being focused only on the genitals

  • Some people describe them as more emotional, intense, or longer lasting

  • Ejaculation may decrease significantly, become clear/white in colour, or stop completely

Shrinkage

(commonly begins around 3–6 months)

  • Testicles may shrink significantly in size

  • The penile shaft may also become smaller over time

Fertility

(changes may begin within 3–6 months)

  • Hormone therapy can reduce fertility and sperm production

  • Some people regain fertility after stopping HRT, while others may not

  • Because these changes can sometimes be permanent, fertility preservation is often recommended before starting hormone therapy
